What is the RPR Test?

Syphilis is a sexually transmitted disease (STD) caused by the bacterium Treponema pallidum. The RPR test is used for detecting the infection by searching for the antibodies formed against it in the blood. An antibody test can be done along with the RPR test to check for an active syphilis infection and also for the prescription of a suitable treatment. This helps in limiting the occurrence of complications and the possible transmission of the disease.

The development of syphilis can be traced through different stages:

  • Primary syphilis: The primary stage begins 2-3 weeks after contracting the infection. One might develop painless chancres in the regions where the bacteria had entered, such as the penis or vagina. You may not recognise them, especially if they are in the rectum or on the cervix. Even without therapy or treatment, they typically go away in 4-6 weeks.
  • Secondary syphilis: An untreated primary stage syphilis infection can develop into secondary syphilis. Symptoms are visible within 6 weeks to 6 months after the appearance of chancre. Skin rashes that are red and non-itchy can also occur on the palms of the hands and the soles of the feet. Other symptoms may include fever, lethargy, enlarged lymph nodes, a scratchy throat, and body aches.
  • Late or tertiary syphilis: An untreated secondary stage syphilis infection can develop into the late or tertiary stage. The infected person, in this stage, can be asymptomatic. The infection can also last for many years and if untreated, can damage the heart, eyes, brain, central nervous system (neurosyphilis), bones, joints, etc, and also result in mental illness, blindness, and even death.
When is the RPR test performed?
  • To check for syphilis in those who might be at a risk of getting the infection (those already diagnosed with another STD; homosexual men who had sex with someone infected with syphilis; those who have unprotected sex, etc.)
  • To check for syphilis in women who are pregnant.
  • To check the efficacy of the treatment.
How to prepare for the RPR test?

There are no preparations required for the RPR test. However, before the test, do inform your doctor:

  • About any medications you’re taking, particularly blood thinners.
  • If you are allergic to any medicine.
  • If you suffer from any bleeding disorder.
  • If you’re pregnant.
How is the RPR test performed?

A blood sample is drawn via an injection from a vein located in your arm by a trained phlebotomist for conducting the RPR test. Alcohol might be used to clean the area from where the blood will be taken. A gauze pad is then used to cover the same.

Normal range: Negative (“non-reactive”)

A positive RPR test means that you have an active syphilis infection. On the other hand, a negative RPR test means that you don’t have an active syphilis infection.

The RPR test can give a false negative result if you are in the last stage of the infection. Therefore, you might be asked to take another test after a couple of weeks have passed.

Similarly, the RPR test can also give false-positive results when you already have antibodies produced against a disease in your body that are similar to the ones produced when you have syphilis.

False-positive findings are possible if you have any of the following:

  • HIV
  • Tuberculosis
  • Systemic lupus erythematosus
  • Pneumonia
  • Used IV drug
  • Lyme disease
  • Malaria
  • Weakened immune system

A doctor will confirm the false-positive result before prescribing you any treatment using another test known as the fluorescent treponemal antibody-absorption (FTA-ABS). This one particularly looks for antibodies produced in your body against the syphilis-causing bacteria.
